Creating Enquiry Forms (Lead Capture Forms)

Enquiry forms allow parents to submit their details online. Every time a parent fills and submits the form, a new enquiry (lead) is automatically created in Illumine. This helps you collect admissions enquiries without manually entering information.

How to Create an Enquiry Form

  1. Go to Enquiries from the left panel.

  2. Click Enquiry Forms.

  3. Click Create Form.

You will now be able to configure the form based on the information you want to collect from parents.

Select the Information to Collect

You can choose which details should be included in the form, such as:

  • Child information (name, age, program, etc.)

  • Parent information (mother/father details, contact info)

  • Custom fields (for example: preferred start date, notes, special requirements)

You can also:

  • Mark certain fields as required

  • Add additional custom questions based on your centre’s needs

This ensures you collect all the necessary information from parents during enquiry submission.

Configure Form Settings

Before saving the form, set up the form preferences:

  • Form Name: The title parents will see when they open the form

  • Default Status: Choose which status the enquiry should be assigned when the form is submitted (e.g., New or Interested)

  • Notify Staff: Select staff members who should receive a notification when a new enquiry is submitted

  • Email Confirmation: Enable if you want parents to receive an email after submitting the form

  • Redirect Page: Choose where parents should be redirected after submission (for example, a thank-you page)

  • Form Status: Set the form to Live so parents can start submitting responses

Once everything is configured, click Save.
Your enquiry form is now ready to use.

How to Share an Enquiry Form

After creating the form, you can share it with parents in multiple ways depending on how you collect enquiries.

Option 1: Share via Link

  1. Go to Enquiry Forms

  2. Click the three dots next to the form

  3. Click Copy Link

  4. Share the link via WhatsApp, email, or SMS

When parents click the link, they can fill in the form and submit their details. A new lead will automatically appear in Illumine.

Option 2: Share via QR Code

  1. Click the three dots next to the form

  2. Select View QR Code

  3. Download and print the QR code

  4. Place it at the reception, entrance, or notice board

Parents can scan the QR code using their phone and submit the form instantly.

Option 3: Embed on Your Website

You can also embed the enquiry form on your school’s website.

  1. Click the three dots next to the form

  2. Copy the iframe code

  3. Share it with your website developer

  4. The developer can embed the form on your website

Parents visiting your website can fill out the form directly, and all submissions will automatically create new enquiries in Illumine.

Using enquiry forms helps streamline your admissions process by capturing leads in real time and ensuring no enquiry is missed.
